How It Works

The Prostate Gland 
The prostate is a reproductive gland located in the male pelvis. Approximately the size of a walnut, it is found behind the pubic bone, in front of the rectum, and below the bladder, surrounded by the pelvic muscles. The prostate surrounds the urethra, the tube that carries urine from the bladder to the penis. The prostate produces a thin, milky alkaline fluid that contains citrate ion, calcium, phosphate ion, a clotting enzyme, and a profibrolysin. During sexual activity, this fluid mixes with seminal fluid and enhances the motility and fertility of the sperm by maintaining optimum pH. The contraction of the smooth muscle of the prostate promotes semen expulsion during ejaculation. Testosterone, estrogen, and related hormones have major roles in the growth and function of the prostate. 
 
Benign Prostatic Hyperplasia 
Benign prostatic hyperplasia (BPH) is an age-related, non-cancerous enlargement of the prostate gland. Changes in the prostatic tissue result in enlargement of the gland. When the prostate enlarges, it may obstruct urinary outflow by compressing or distorting the urethra. As the obstruction increases, frequent urination, nocturia (urination during the night), difficulty starting or maintaining a stream of urine, dribbling of urine, urinary retention, and incontinence (unintentional loss of urine) may occur. While the exact cause is unknown, the fact that BPH occurs in older men suggests a relation to changes in the hormonal balance associated with aging. 
 
Prostate Cancer 
Prostate cancer is the most commonly diagnosed cancer in American men. An estimated 180,400 new cases, with an estimated 31,900 deaths, will occur in the United States during the year 2000. With aging, the risk of prostate cancer increases. As the average age of the American male continues to increase, so likely will the incidence of prostate cancer. While prostate cancer is found worldwide, disparities may be found in certain racial and ethnic groups. The incidence for prostate cancer is 37% higher for African American men than for Caucasian American men. Japanese men have the lowest incidence and mortality rates from prostate cancer. 
 
While the exact cause of prostate cancer is unknown, certain risk factors have been identified. Diets high in animal fats may alter the hormonal environment and predispose a man to an increased prostate cancer risk. Serum testosterone levels may also be related to the development of prostate cancer. African American men have an average 15% higher serum testosterone level than white males. Exposure to carcinogens such as cigarette smoke and a high percentage of body fat may also play a role in prostate cancer. 
 
How Does It Work?: 
Metabolism of active foreign substances or detoxification is a process that requires enzymes. The enzymes involved in detoxification are generally divided into two groups, known as Phase I and Phase II enzymes. Phase I enzymes, such as cytochrome P450-dependent monooxygenase, convert hydrophobic (fat-soluble) compounds to electrophilic (active) derivatives. Phase II enzymes, which include glutathione transferase, glucuronosyltransferase, and sulfotransferase catalyze conjugation reactions. Most reactions catalyzed by Phase II enzymes generally produce high polar molecules that are readily excreted from the body. While the Phase I reactions tend to be an activating process, the Phase II reaction is more likely to be a detoxification process. 
 
Glucuronidation is a major pathway of the Phase II detoxification process. It is catalyzed by glucuronosyltransferase and requires the cofactor UDP-glucuronic acid. When glucuronidated, these compounds become polar, water-soluble conjugates that are eliminated from body in urine and bile. However, active compounds conjugated with glucuronic acid are substrates of β-glucuronidase. β-glucuronidase can reconvert substances back to their Phase I, potentially harmful form, which reabsorbs into the body and delays the elimination of these compounds. 
 
Calcium D-glucarate (CGT) is a precursor of glucarolactone (GL), which inhibits β-glucuronidase. By inhibition of β-glucuronidase, GL increases the amount of these compounds sequestered and excreted as the glucuronides, thus decreasing the portion of active compounds that could be hazardous to the body. In animal studies, administration of CGT was reported to inhibit β-glucuronidase activity. CGT was also found to support healthy cell development. 
 
Broccoli is a vegetable from the genus Brassica. It contains a large number of phytochemicals, some of which have been shown to be beneficial. In the early 1990s, broccoli was found to increase the activity of Phase II enzymes. Later, sulforaphane was identified as the principal Phase II enzyme inducer in broccoli extract. In animal studies, sulforaphane was reported to enhance chemical detoxification and support healthy cell development. 
 
Green tea is a dried product of the fresh shoot of the tea plant (Camellia sinensis). Green tea is a rich source of flavonoids and polyphenols, namely, catechins and flavonols. The catechins, also known as tea polyphenols, which occur in significant quantities in green tea, are catechin, epicatechin, gallocatechin, epigallocatechin, epicatechin gallate, and epigallocatechin gallate (EGCG). Animal studies have shown that the consumption of tea and its polyphenols afford health supportive benefits. In addition, green tea constituents, EGCG and ECG are potent 5-α-reductase inhibitors. The proposed mechanism of action for green tea and its constituents may be associated with antioxidant and free-radical scavenging activity, stimulation of detoxification systems, and the support of healthy cell development. Furthermore, epidemiological studies suggest that regular tea consumption supports prostate health. 
 
Maitake mushroom (Grifola frondosa) is an edible mushroom consumed in China and Japan both for its taste and health benefits. Recently, its powder and water extracts have been investigated for effects on the immune system and to support healthy cell development. Maitake D-fraction is a mixture of β-D-glucan fraction obtained from maitake water extracts. It mainly contains α-1,6' D-glucan with α-1,3' branches. In an animal model, D-fraction was shown to activate macrophages, natural killer cells and cytotoxic T-cells with a concomitant increase in interleukin-1 production. In the spring of 1998, an Investigational New Drug Application (IND) was obtained from the Food and Drug Administration (FDA) for the use of D-fraction in a phase II pilot study. 
 
Lycopene is the major carotenoid in tomatoes and tomato products. It gives tomatoes their characteristic color. Consumption of lycopene rich food, as well as higher concentrations of blood lycopene, is associated with a healthy prostate. Additionally, lycopene is found in the human prostate, which suggest the biological plausibility of a direct support of this carotenoid on prostate function. The mechanism by which lycopene might support prostate health remains unclear. Lycopene is a potent antioxidant and efficiently neutralizes single oxygen and free radicals. It also enhances the expression of gap-junction proteins involved in cell-cell communication. Finally, lycopene may protect cellular components such as DNA from specific types of damage from free radicals. 
 
Selenium is an essential trace element, which is found in food. It is a key component of glutathione peroxidase, which removes hydrogen peroxide and lipid hydroperoxides generated by free radicals and oxygen derived species. When selenium is deficient, the activity of glutathione peroxidase will be impaired, which may lead to accumulation of lipid hydroperoxides. Lipid hydroperoxides are not stable. They may damage cell membranes, DNA, and other cellular components. Therefore, optimal intake of selenium is important to protect the body from oxidative damage. In humans, higher selenium intake was reported to support prostate health. Supplementation of selenium has been shown to promote prostate health.
