Diet pill’s icky side effects keep users honest
Threat of embarrassment forces Alli users to eat healthy — or else

By Melissa Dahl - Health writer - MSNBC

Alli is the first over-the-counter weight loss drug to get an approval from the Food and Drug Administration, but it can cause some ghastly side effects if users cheat on their diets.

CBS NEWS:

All About Alli, The Weight Loss Pill.
(WebMD) By now, you have probably seen the commercials or read the direct mail pieces about Alli. They ask if you are ready to commit to a weight loss plan that includes Alli, the over-the-counter version of the FDA-approved fat blocker formerly known as Xenical.

FDA APPROVED:

The Food and Drug Administration approved the drug product orlistat 60 mg capsule (trade name alli) on February 7, 2007, for over-the-counter marketing as a weight loss aid. Alli is to be used only in conjunction with a weight loss program that includes a reduced calorie diet, a low fat diet, and an exercise program. It is approved for use in adults 18 years and older. A multivitamin should be taken every day when alli is used as part of a weight loss program.

SUGGESTED USAGE
For overweight adults 18 years and older:
  • take 1 capsule with each meal containing fat
  • do not exceed 3 capsules daily
  • use with a reduced-calorie, low-fat diet until you reach your weight loss goal. Most weight loss occurs within the first 6 months
  • if you stop using alli, continue with a diet and exercise program
  • if you start to regain weight after you stop taking alli, you may need to start taking alli again along with your diet and exercise program
  • take a multivitamin once a day, at bedtime, when using alli capsules
When using this product
  • take a multivitamin once a day, at bedtime. alli can reduce the absorption of some vitamins
  • follow a well-balanced, reduced-calorie, low-fat diet. Try starting this diet before taking alli capsules.
  • alli capsules work by preventing the absorption of some of the fat you eat. The fat passes out of your body so you may have bowel changes.
You may get:
  • gas with oily spotting
  • loose stools
  • more frequent stools that may be hard to control
  • eating a low-fat diet lowers the chance of having these bowel changes
  • for every 5 pounds you lose from diet alone, alli can help you lose 2-3 pounds more. In studies, most people lost 5-10 pounds over 6 months
Warnings:

Organ transplant alert:
  • do not use if you have had an organ transplant. alli interferes with the medicines used to prevent transplant rejection
Allergy alert:
  • do not use if you are allergic to any of the ingredients in alli capsules
Do Not Use
  • if you are taking cyclosporine
  • if you have been diagnosed with problems absorbing food
  • if you are not overweight
Ask a doctor before use if you have ever had
  • gallbladder problems
  • kidney stones
  • pancreatitis
Ask a doctor or pharmacist before use if you are
  • taking warfarin (blood thinning products), or taking medicine for diabetes or thyroid disease. Your medication dose may need to be adjusted.
  • taking other weight loss products
Stop use and ask a doctor if
  • severe or continous abdominal pain occurs. This may be a sign of a serious medical condition
If pregnant or breast-feeding, do not use.

Keep out of reach of children.
In case of overdose, get medical help or contact a Poison Control Center right away.

PLEASE NOTE: Alli must be consumed with a multi vitamin nightly to ensure adequate absorbtion of Vitamins A, D, E, K, and beta carotene.

How It Works

alli in the body
You're probably curious about what alli does in your body. The active ingredient in alli attaches to some of the natural enzymes in the digestive system, preventing them from breaking down about a quarter of the fat you eat. Undigested fat cannot be absorbed and passes through the body naturally. The excess fat is not harmful. In fact, you may recognize it in the toilet as something that looks like the oil on top of a pizza.

Because of the way in which alli works, eating meals with too much fat may cause side effects known as treatment effects. Learn more about treatment effects now.

alli only works on enzymes that break down fat, not on other enzymes. So carbohydrates and proteins are not affected. You still absorb those nutrients. But, since fat grams have more than twice as many calories as carbs and proteins, alli capsules work on the worst offenders - the calorie-dense fat grams.

alli is safe when used as directed - it works only in your digestive system, not in your heart or brain. That means you won't feel:
  • jittery effects
  • sleeplessness
  • a racing heart
FAQ

How does the alli program work?

Alli is the only FDA approved, over-the-counter weight loss product. But it's more than just a pill. It's an innovative weight loss program.

The pill works by preventing your body from absorbing some of the fat you eat. The active ingredient in alli attaches to some of the natural enzymes in the digestive system and prevents absorption of about a quarter of the fat you eat. Undigested fat cannot be absorbed and passes through the body naturally. Because alli works only in the digestive tract, it will not affect your heart or brain so you will not experience any racing heart or jitteriness.

Along with the capsule, the alli program supplies you with a wealth of education, advice and support.

How effective is the alli program?
With hard work, you can lose about 50% more weight than with just dieting alone. That means if you could lose 10 pounds through dieting alone, you could lose 15 with alli. Those who have been successful with alli have been dedicated to changing their eating and activity habits as outlined in the alli program.

If alli capsules only work on the fat in my food, how do I lose the fat in my body?
As you consume fewer calories on a reduced-calorie, low-fat diet — and use alli to block absorption of some of the fat you eat — your body will automatically tap into its fat stores and use them for the energy it needs. That can result in weight loss.

How much weight can I expect to lose?
That depends on how closely you follow a reduced-calorie, low-fat diet, get regular physical activity, and take alli capsules as directed. alli promotes gradual, modest weight loss of about 1 to 2 pounds a week. If you could lose 10 pounds through dieting, you could lose 15 with hard work and alli. Results for each individual may vary.

How do I know when I'm ready for the alli program?
You may be ready for alli if you can answer "yes" to these conditions:
  • I am willing to do the hard work to lose weight gradually
  • I am committed to following a reduced-calorie, low-fat diet (an average of 15 grams of fat per meal)
  • I understand that if I take alli with a meal containing too much fat, I may get bowel changes known as treatment effects. The effects may include gas with oily spotting, loose stools, and more frequent stools that may be hard to control. Why? Because alli prevents absorption of some of the fat, and undigested fat passes through the body
  • I am committed to eating smaller portions
  • I am committed to making time to be more physically active
  • I will read and follow the alli label
How soon after taking alli capsules should I expect to see results?
If you follow a reduced-calorie, low-fat diet, get regular physical activity, and take alli capsules as directed, you should see results in the first two weeks. Most weight loss usually occurs within the first six months. If you stop using alli, continue with a diet and exercise program for help with weight loss. You may start to regain weight after you stop taking alli. In order to maintain your weight loss, you may need to continue taking alli along with a reduced-calorie, low-fat diet.

What can I eat on the alli program?
When you partner with the alli program, you’ll be able to eat meals that are both healthy and delicious. With the help of our healthy eating guide and interactive meal planner, you’ll find yourself indulging in flavorful meals and snacks every day of the week. And that’s important, because part of your success depends on how well you commit to a reduced-calorie, low-fat diet while taking alli capsules as directed.

The alli program is not about starvation or deprivation; so while it’s important to limit choices that are high in fat and/or calories, you will find that you can still enjoy many of your favorite foods in moderation.

Should I use a multivitamin with alli?
Take a multivitamin once a day at bedtime while using alli capsules to ensure adequate absorption of vitamins A, D, E, K, and beta-carotene. We recommend Centrum, a balanced and complete multivitamin.

How is alli different from other weight loss products?
alli is not a dietary supplement. It is an FDA approved weight loss product. Orlistat (the active ingredient in alli) has been extensively studied and safely taken by millions of people worldwide. Americans spend an estimated $33 billion on weight loss products and programs that in some cases are unregulated and unsafe. The absence of a proven safe and effective OTC product for weight loss has been a void in the nation's arsenal of weapons to fight the overweight and obesity epidemic. Orlistat is the only weight loss product approved by the FDA to go over-the-counter. Orlistat can help people lose 50% more weight than with dieting alone. That means if you could lose 10 pounds with a diet, you could lose 15 pounds working with alli. Individual results may vary.

How effective is alli?
alli works by blocking the absorption of about 25% of fat in the foods you eat, leading to a 5% to 10% weight loss over time. alli met the rigorous FDA standards for a weight loss product. Clinical trials with alli have consistently shown it to be more effective than diet alone. alli can help people lose 50% more weight than with dieting alone. This means that if you could lose 10 pounds with a diet, you could lose 15 pounds working with alli.

Is there a benefit in losing 5% to 10% of your weight?
Obesity experts in the U.S. agree and widely available research demonstrates that moderate weight loss is beneficial. It is important to emphasize the health benefits of 5% weight loss and to change the perception of Americans who reject this as an insignificant amount of weight loss. Achievable, realistic weight loss of 5% can result in significant improvements in overall health, including decreased risk for heart disease. Slow, steady weight loss is the healthy way to reduce body fat mass. The alli program encourages the lifestyle changes that help achieve realistic goals of improved health and weight loss maintenance.

The math behind alli does not seem to add up. If the drug in the alli capsule inhibits the absorption of about 25% of the fat that I eat, and I follow the plans, it would take me around 5 weeks to lose 1 pound. How does that equate to 50% more weight loss than dieting alone?
It is important to remember that alli capsules are not intended to do all of the work for you or to replace diet and exercise. alli is indicated to be taken with a reduced-calorie and low-fat diet.

Clinical studies have proven that people who followed a reduced-calorie, low-fat diet while taking alli capsules experienced 50% more weight loss than those who just followed the diet. alli capsules (orlistat 60 mg) prevent absorption of about 25% of the fat you eat from being absorbed and cause a calorie deficit in your diet. The actual amount of fat blocked by alli is dependent on the total amount of fat consumed. As discussed in the educational materials that are provided in the alli starter pack, individuals will have different total caloric daily needs based on their weight and activity levels.

In addition, individuals who used alli reported experiencing greater motivation. They were more likely to follow and maintain a low-fat diet while on alli. In addition, they were more likely to exercise.

Who can take alli?

alli is not for everyone. alli offers individuals who are ready and committed to make the lifestyle changes necessary for safe, realistic, and sustained weight loss, with the added incentive to lose more than with diet alone. It can be taken by overweight individuals age 18 or older who are committed to adhering to a reduced-calorie, low-fat diet. But, as with all medicines, there are some people who should not take alli or who need to check with the doctor first, so it is important to review the label before purchasing alli.

alli has been studied in ethnically diverse populations and has been proven safe and effective for all populations regardless of gender, race, and initial weight. Data shows that alli plus diet is always significantly better than diet alone.

Can children use alli?
No. alli is indicated for use by overweight individuals 18 years of age or older.

How should alli be taken?
The recommended dose of alli is 1 capsule 3 times a day with meals containing fat. Additionally, to ensure adequate vitamin absorption, those taking alli should also take a multivitamin once a day, at bedtime, because alli may reduce the absorption of fat-soluble vitamins (vitamins A, D, E, K, and beta-carotene).

Is Xenical the same as alli?
Xenical and alli contain the same active ingredient (orlistat), but they are not the same. The products have different indications and dosages. Xenical, manufactured by Roche, is prescription orlistat, a 120 mg capsule indicated for the treatment of obesity in patients who require the supervision of a physician.

alli is 60 mg orlistat, an over-the-counter weight loss product for overweight adults who follow a reduced-calorie, low-fat diet. As an FDA approved OTC product, alli puts the consumer in control by offering a proven OTC weight loss product with comprehensive support.

When do I take alli?
The mealtime dosing schedule (one hour before, during, or one hour after a meal) makes it easy to remember alli capsules. To help you remember, we recommend making alli a part of your pre-meal routine. Once alli and the routine have been paired several times, the association will become automatic. However, if a dose is missed, alli can be taken up to one hour after a meal and still be effective. The alli Shuttle™ carrying case is provided with the alli starter pack to enhance compliance and serve as a helpful reminder to take alli capsules with your meals.

If I eat more than 3 meals a day, should additional doses of alli be taken?
It is recommended that alli be taken with meals — up to 3 times a day. Additional doses are not recommended.

If I use alli more than 3 times daily, will I experience better results?
No. Dose ranging studies show that it is unlikely that higher doses will provide better results. Additional doses are not recommended.

How long can I take alli?
The alli label does not limit the amount of time you can take alli. You can take it until you achieve the success you want with weight loss. Most weight loss occurs within the first 6 months of using alli.

If you stop taking alli capsules, continue with your diet and exercise program to help maintain your weight. If you start to regain weight after you stop taking the capsules, you may need to start taking them again along with a diet and exercise program.

Can a person drink alcohol while taking alli?
Yes. There is no interaction between alli and alcohol. However, alcohol is high in calories. It can, therefore, contribute to weight gain.

What should a woman do if she becomes pregnant while taking alli?
Women who are pregnant or breast-feeding should not take alli. If a woman discovers that she is pregnant while taking alli, she should stop taking the capsules and consult with her physician about appropriate body weight and diet while pregnant or breast-feeding.

What happens if alli is taken with a fat-free meal?
Nothing will happen because there is no fat for alli to inhibit. alli only works locally in the gut on GI lipase activity and does not cause any bowel changes without the consumption of dietary fat.

How do I best pick a starting date to begin taking alli?
Pick the day you will begin taking alli capsules, and start the diet a few days before you begin taking the capsules. If possible, start taking alli capsules on a weekend when you are home.

I don't like to eat breakfast. Can I just pass on that meal?
Clinical studies show alli works best when you eat 3 healthy, low-fat meals a day, one of which is breakfast. If you skip a meal or you are certain your meal contains no fat, you can skip a dose.

People who skip breakfast tend to overeat later in the day. There are plenty of quick, satisfying breakfast choices in the alli meal plans.

Is there a waiting limit between taking 2 capsules?
The recommended dose of alli is 1 capsule with meals containing fat, and no more than 3 capsules daily. alli should be taken within an hour of each meal containing fat. It's best to wait at least 2 hours in between doses. However, it is also best to spread your calories throughout the day in breakfast, lunch, and dinner.

Is dieting necessary while taking alli?
Yes. A well-balanced, reduced-calorie, low-fat diet containing about 15 grams of fat per meal is recommended while taking alli. These recommendations are consistent with dietary guidelines established by the USDA (total fat intake between 20% to 35% of calories).

Purchase of alli includes a comprehensive educational support program and an individually tailored, online action plan to help ensure optimal use of alli. Materials include Welcome and Companion Guides, a "Read Me First" Guide, a Guide to Healthy Eating, a Daily Journal, a Calorie & Fat Counter, and Quick Facts Cards.

Can alli be used with diets that are low in carbohydrates and high in fat?
No. alli is only recommended in conjunction with low-fat diets that contain about 15 grams of fat per meal. Eating a meal with too much fat while taking alli can result in bowel changes, such as loose stools, more frequent stools that may be hard to control, or gas with oily spotting. These changes, called treatment effects, may occur in the first weeks of treatment, are not harmful, and can be managed by following a reduced-calorie, low-fat diet of 15 grams of fat per meal.

What happens if I forget to take alli capsules?
If you miss a dose, you simply lose the benefit of alli capsules to block absorption of some of the fat in foods you eat for that meal. Resume taking alli at your next meal.

Does alli begin to work in the stomach, or does it begin to work in the small or large intestines?
alli does begin to work in the stomach, but the majority of the action occurs in the small intestine.

Is alli safe?
alli has been approved by the FDA as safe and effective when taken according to label directions. The safety and efficacy of orlistat are both well established. A higher dose of orlistat has been marketed as the prescription drug Xenical in the U.S. since 1999 and is supported by nine years of use in 145 countries. Orlistat experience in more than 25 million patient treatments and more than 100 clinical studies with more than 30,000 patients is unprecedented for a weight loss drug. One of these studies was the four-year landmark XENDOS trial conducted by its manufacturer, Roche. This study is the longest study ever conducted for a weight loss drug.

Taken at mealtime, alli works by blocking the absorption of ¼ of the fat in the foods you eat. alli is non-systemically acting, which means it does not affect the central nervous system (CNS) and has no effect on the heart or the brain.

Does alli cause any heart racing or palpitations?
No. alli works in the gut; it is non-systemically acting, which means it does not affect the central nervous system (CNS) and has no effect on the heart or the brain.

What is the rationale behind the label advice for patients with gallbladder disease, pancreatitis, and kidney stones to speak with their doctor?
Obesity puts people at a greater risk for these conditions. If you have a history of any of these conditions, talk to your doctor before using alli.

Can you take alli if you have had your gallbladder removed?
If you have had your gallbladder removed, you may have problems absorbing food. Check with your doctor before using alli.

Can you take alli if you have had gastric bypass surgery?
If you have had gastric bypass surgery, you should talk with your doctor before taking alli. Some patients experience problems absorbing food after such a procedure. If this is the case, alli may not be right for you. However, everyone handles these procedures differently, so it is best to check with your doctor first.

How long does alli remain in your system?
Since everyone has different GI transit times, the active ingredient in alli can actually remain in your system for up to 48 to 72 hours, mainly because gastric emptying time varies from person to person.

Will alli interfere with birth control medications?
No. alli does not impact the efficacy of your birth control medication. A clinical trial using orlistat (the active ingredient in alli) in 20 normal body weight women taking birth control pills for 23 days found that orlistat had no effect on the ovulation suppression action of the birth control drug.

Can you take antibiotics while taking alli?
Yes. There are no clinically significant drug interactions with antibiotics.

If alli prevents the absorption of some of the vitamins I take, will it also block the medication that I take for cholesterol?
No. It won't block a cholesterol medication.

will alli block any other medications besides those for cholesterol?
alli may block other medications. Please check the alli label before purchasing alli. Please consult with your healthcare professional.

Does alli interfere with bile acid sequestrants such as cholestyramine (Questran), colestipol (Colestid), and colesevelam (Welchol)?
Since these medications often bind, and therefore inactivate, other drugs given concurrently, it is recommended that you take alli at least 1 hour before or 4 to 6 hours after bile acid sequestrant administration to avoid impeding the effects of alli.

Do alli and Zetia work the same way? Can I take them both together?
Zetia reduces blood cholesterol by inhibiting the absorption of cholesterol in the small intestine. alli, on the other hand, works in the stomach, where it inhibits the action of gastric and pancreatic lipases. Since these two drugs have different mechanisms of action and work at different sites in the body, it is unlikely that there will be an interaction between the two.

Is it OK for diabetics to use alli?
Ask a doctor before using alli if you are taking medicines for diabetes. There is no drug interaction between alli and diabetes medications; however, people with diabetes may have lower blood sugar levels and improved glycemic control after losing weight with alli. This may require a dose reduction of their diabetes medication.

I am also on hypertensive medications. Will I be able to take alli?
alli does not interact with hypertensive medications, so it is OK to take alli while taking hypertensive medications. If you have questions, please check with your healthcare professional.

Can I take alli if I am taking medicine for thyroid disease, e.g., Levoxyl for hypothyroidism?
Check with a doctor or pharmacist before taking alli if you are taking medication for thyroid disease. People being treated for thyroid disease may need the dose of their thyroid medication adjusted based on their weight loss.

Is it safe to take alli with the migraine medications Topamax and Maxalt?
It is safe to take alli with the migraine medications Topamax and Maxalt. But, if you have any medical condition, you should first consult a physician before you change your dietary habits.

Is alli safe for those taking cyclosporine (Neoral, Sandimmune, SangCya)?
The label warns consumers in two label sections (organ transplant patients and cyclosporine users) that they should not use alli because it can lower the levels of that drug in the body.

Is alli safe for those taking cyclosporine eye drops?
Do not use alli if you are taking cyclosporine. If you have any other questions, check with your doctor first.

Is alli safe for those taking warfarin (Coumadin)?
If you are using warfarin, check with your doctor or pharmacist before taking alli, as you will need to be monitored and may need to have your warfarin dose adjusted.

Can alli be taken with Benecol?
Yes. There has been no interaction identified between the ingredient in Benecol (plant stanol esters) and orlistat (the active ingredient in alli). On the alli program, you should aim for 15 grams of fat or less from each meal. One serving of Benecol Spread or Benecol Light Spread contains 8 or 5 grams of fat, respectively. Therefore, patients need to be mindful of the contribution of the fat in Benecol to the total amount of fat in the meal.

Can you take alli if you have irritable bowel syndrome?
You should first talk to your doctor before purchasing alli if you have IBS. People with IBS have problems absorbing food. Do not use alli if you have problems absorbing food.
